AOP Orphan aims to conduct a clinical trial in patients suffering from COVID-19 in order to deter-mine the safety and efficacy of Selisistat, a highly selective inhibitor of silencing information regula-tor T1 (SIRT1). Selisistat (Ex-527) targets the angiotensin-converting enzyme 2 (ACE2), the main SARS-CoV-2 receptor required for entry and effective replication of the virus in the host cell. Since the safety of Selisistat (racemic mixture of S-[active]- and R-[inactive] enantiomers) was investigat-ed in several clinical trials, the drug was found to be safe and well tolerated in healthy subjects and patients. AOP Orphan aims to conduct a phase II study with the already available drug (racemic mixture in an oral tablet). The clinical trial will be initiated with 40 patients for a safety review to evaluate the use of Selisistat in COVID-19. In parallel, AOP plans to optimize the drug formulation (active S-enantiomer without the R-[inactive] enantiomer) for COVID-19 treatment. The optimized drug candidate will be evaluated in the second part of the phase II trial for safety and efficacy.
