Projektbeschreibung

Die Langzeitpflege und -betreuung älterer zu Hause lebender Menschen steht vor großen Herausforderungen, sowohl auf der Angebots- (Pflegekräftemangel) als auch Nachfrageseite (steigende Anzahl pflegebedürftiger Menschen). Da ältere Menschen es vorziehen zu Hause alt zu werden und auch um Kosten zu sparen, hat die Politik in vielen Ländern der Pflege und Betreuung zu Hause politische Priorität gegenüber stationären Einrichtungen eingeräumt. Mit zunehmendem Alter und sich verschlechternden Gesundheitszustand wird die häusliche Pflege auch mit professioneller Unterstützung immer anspruchsvoller.
Daher hat sich „Care about Care“ (C^C) zum Ziel gesetzt, das Ökosystem der professionellen Hauskrankenpflege (Pflegekräfte, pflegebedürftige Menschen, pflegende An- und Zugehörige, Pflegeorganisationen, Anbieter von Software- und AAL-Lösungen) zu unterstützen, indem neue Wege in der Zusammenarbeit und beim Informationsaustausch beschritten werden. Durch die Entwicklung sowie Kombination von IKT-Lösungen soll:
a) Neu Möglichkeiten der „remote“ Zusammenarbeit und Unterstützung von informell und formell Pflegenden, durch erfahrene Pflegekräften und Spezialisten (z.B. Wundmanager, Therapeuten) geschaffen,
b) Der Zugang und Austausch von Informationen zwischen Mitgliedern des Pflegenetzwerks verbessert und
c) Der Wissenstransfer zwischen Experten Pflegepersonal, Pflegeempfängern und pflegenden Angehörigen gefördert werden.
Aufbauend auf vorhandenen Systemen für die Hauskrankenpflege (es werden offene Schnittstellen geschaffen) werden zwei neue Dienste entwickelt und mit bestehenden und neuen organisatorischen Prozessen kombiniert, um:
• Ein Service zur „remote“ Unterstützung von Pflegekräften und informell Pflegenden zu entwickeln, welches ExpertInnenwissen bereitstellt und neue Arbeitsabläufe innerhalb der Pflegeorganisationen ermöglicht, um erfahrene MitarbeiterInnen zu halten und junge Menschen für die Pflege zu gewinnen
• Ein „Care Cockpit“ bereitzustellen, welches als Informationszugangs- und Austauschportal für Pflegeorganisationen, ihre Kundinnen und informell Pflegende dient.
Um eine möglichst hohe Bedienbarkeit und Akzeptanz zu gewährleisten, werden alle am Pflegeprozess Beteiligten in einzelne Projektphasen involviert. Um dies zu ermöglichen wird C^C in drei Iterationen entwickelt und evaluiert. Die Lösung entwickelt sich so sukzessive hinsichtlich Benutzeranforderungen, Funktionalität, technischer Reife und Marktpotential weiter. Um während der 3. Iteration aussagekräftige Evaluierungsergebnisse zu bekommen, wird eine randomisierte kontrollierte Studie in drei Ländern durchgeführt. Die begleitende Evaluierung (Usability, Akzeptanz, Funktionsfähigkeit und Wirkungen) soll Aufschlüsse über Marktbarrieren und Markttauglichkeit geben.

Abstract

Long-term care and support of community-dwelling older adults faces severe challenges – on both the supply (e.g. shortages of formal and informal carers) and demand side (e.g. increasing number of care-dependent people). As older people prefer to stay at home and to contain costs, policies in many countries give home care services political priority over residential care. With increasing age and deteriorating health conditions, caring at home becomes a more and more demanding task even with the support of professional home care services.
‘Care about Care’ (C^C) thus aims to support the ecosystem of professional home care settings (care workers, care recipients, family, non-kin carers, care organizations, software and AAL service providers) by providing new ways of collaboration and information exchange. By developing and combining ICT-based solutions C^C intends to:
a) initiate new ways of collaboration and support of both family carers and care workers onsite by remote advice and support from experienced graduate nurses and specialists (e.g. wound managers, therapists)
b) improve information access & exchange and tools to better monitor service provision and costs, and facilitate swift exchange between members of the care network
c) promote knowledge transfer between experts/specialists, care workers, care recipients and family carers.
By building on existing software solutions for professional home care and complementing them with additional newly developed services (using open interfaces), two ICT-enhanced services will be created and combined with established and new organizational processes:
• “C^C Remote Care Assist” service professionally supports and advises care workers and informal carers by providing expert knowledge from distance and initiate new workflows that help to retain experienced staff and attract young people into care work.
• “C^C Care Cockpit” is the information access & exchange portal for care organisations, their clients and informal carers
To ensure highest possible usability, user experience and acceptance, all members of the care network will be involved in relevant project phases. C^C will be developed and evaluated in three iterations which means that the solution will evolve in terms of user needs, functionality, technical maturity and business aspects. In the 3rd iteration, a randomized controlled trial will be conducted in three countries. The results of the accompanying evaluation (usability, acceptance, functionality and effectiveness) will contribute identify market barriers and marketability.
