Covid-19 Patienten benötigen bei einem schweren Krankheitsverlauf Beatmungsgeräte, die über mehrere Tage mit Sauerstoff angereicherte Luft in die Lunge pumpen. Solche Geräte sind teuer und bei einer Eskalation der Pandemie nicht in ausreichender Menge vorhanden. Die meisten handelsüblichen Geräte und auch die aus der Notlage heraus von mehreren Gruppen nachgebauten Systeme arbeiten nach dem Volumsprinzip. Das gesamte benötigte Luftvolumen wird aus einem Zylinder (Kolbenpumpe) oder Beutel (Ambu-Beutel) in die Lunge gepresst. Diese Systeme sind daher relativ groß und in der Anwendung nicht unkritisch, weil bereits angegriffene Lungen durch zu hohen Druck und falsche Beatmungsstrategien anhaltend geschädigt werden können.
Im vorliegenden Projekt Spiritum 1.0 wird die Beatmung mittels einer Rotationspumpe umgesetzt, die deutlich kleiner ausgeführt sein kann. Spezielle Software moduliert bei
gleichbleibender Durchflussrichtung die Drehzahl der Pumpe und steuert ein nachfolgendes Umschaltventil zur Regelung der Inspirations- und Expirationsphasen. Dazu ist das System mit Sensoren zur Messung von Drehzahl, Druck und Durchflussmenge ausgestattet. Eine flexible und erweiterbare Software ermöglicht bei gleichzeitig einfacher Bedienung verschiedene Beatmungsmodi und Funktionen (CPAP (continuous positive airway pressure), BiPAP (Biphasic Positive Airway Pressure), PEEP (positive end-expiratory pressure), Atemsynchronisation, usw.). Die Einheit wird zusätzlich mit Sensoren für Luftfeuchtigkeit, Temperatur und Sauerstoffgehalt sowie einer telemetrischen Steuereinheit und einem Alarmsystem ausgestattet.
Das Beatmungsgerät wird als Bausatz konzipiert, sodass ein Nachbau leicht möglich wird. Low-Cost-Teile werden eingesetzt, Pumpe, Umschaltventil und Anschlüsse lassen sich mit billigen 3D-Druckern herstellen. Sensoren, Elektronik, Pumpentreiber, Motoren, Schläuche etc. werden dem Bausatz beigelegt bzw. die Bezugsquellen angegeben. Es wird ein Netzwerk aufgebaut, über das engagierte Personen oder Institutionen im Falle eines Engpasses bei der 3D-Druckfertigung aushelfen können.
Eine weitere Aufgabe des Projektes ist eine Konzepterstellung zur Einschulung von Personen, die die Beatmungssysteme einsetzen werden, sowie die Überführung in eine zusätzliche industrielle Herstellung mit hohen Stückzahlen.
Ein prozessbegleitendes Qualitätsmanagement zur Erfüllung der Sicherheitsstandards für die Basis einer MPG-Zulassung wird angewendet.

Abstract

Covid-19 patients with a severe course of disease must be treated with ventilators for several days. Such equipment is expensive and is not available in sufficient quantities if the Corona pandemic escalates. Most of the state of the art devices and also the systems developed recently due to the corona crises are pressing a certain volume from a tube or bag (ambu-bag) into the lungs. These systems are therefore relatively large. Ventilation with such pumps is critical, as covid-19-affected lungs could be permanently damaged by excessive pressure and incorrect ventilation.
In the present FFG-project Spiritum 1.0 ventilation is implemented by means of a rotation pump (Francis turbine), which can be considerably smaller in size and saver to operate. While the airflow stays in the same direction, a subsequent reversing valve imitates inhalation and exhalation. Special software controls the speed profile of the pump and the state of the valve, managing all individual operation modes (CPAP (continuous positive airway pressure), BiPAP (Biphasic Positive Airway Pressure), PEEP (positive end-expiratory pressure), synchronisation, etc.). For this, we will use a set of sensors to measure rotation speed, pressure, flow, humidity, temperature, and oxygen level. Remote control and alarm features will be added. This high effort ensures that the safety standards of a medical product are met.
The ventilator is designed as an assembly kit, so that interested parties can easily reproduce it. Parts like pumps, valves and connectors can be produced with low cost 3D-printers. Sensors, electronics, motors and tubes will be delivered with a construction manual. A user network will be established to help dedicated individuals or institutions to overcome a bottleneck in 3D printing in case of emergencies.
An additional goal of the project is the development of a concept for the training of persons who will use the ventilation systems and the transfer to an additional industrial
manufacturing with high quantities.
Consultancy services include concept and strategy development, process support and monitoring & evaluation as a basic for medical product.
