Technische Unterstützung bei der integrierten (Nach-)Versorgung älterer Menschen

Projektbeschreibung

Die Möglichkeit für ältere Menschen mit Betreuungs- und Pflegebedarf ein selbstbestimmtes Leben Zuhause zu führen, determiniert ganz wesentlich die Lebensqualität. Dieses Bedürfnis zu unterstützen, ist unzweifelhaft als kommunales Aufgabenfeld im Rahmen der Daseinsvorsorge einzuordnen. Mit Blick auf die demografische Entwicklung ist davon auszugehen, dass im Verlauf der nächsten Jahre und Jahrzehnte die Bewältigung dieser Aufgabe für alle Kommunen zur großen Herausforderung wird. Da Prognosen zufolge die erforderlichen professionellen Betreuungs- und Pflegeressourcen nicht sichergestellt sind und die Angehörigenbetreuung aufgrund geänderter Lebenssituationen (wenn überhaupt) nur zeitlich begrenzt zur Verfügung steht, gilt es verstärkt die Einsatzmöglichkeiten von digitalen, intelligenten Assistenztechnologien zu prüfen.

Dieses Sondierungsvorhaben verfolgt das Ziel, eine innovative digitale Assistenz für die Selbst- und Primärversorgung älterer Menschen zu ermitteln und in einem Lastenheft zu beschreiben. Sie soll im Anschluss in der Stadt Dornbirn auch umgesetzt werden. Die Stadt Dornbirn ist Trägerin eines umfassenden Altenversorgungssystems und bietet über Vereine und Institutionen im ambulanten Bereich (u.a. auch der Krankenpflegeverein) umfangreiche Dienste für ältere Menschen an. Dementsprechend können in die Lösungsfindung sehr viele unterschiedliche primäre und sekundäre AnwenderInnen eingebunden werden. Gesucht wird eine Lösungsidee, die in der Lage ist, die Effizienz und Qualität in der persönlichen Betreuung Zuhause zu steigern, die Aufenthaltsdauer für stationäre Versorgung, Rehabilitation und Nachsorge zu verkürzen, das selbstbestimmte Leben Zuhause zu verlängern, die Gesundheit und Gesundheitskompetenz zu fördern und eine individuelle Fachberatung sowie Selbstmanagement zu garantieren.

Es schwebt uns ein neues intelligentes Soft- und Hardware-System zur Beratung, Planung, Steuerung und Dokumentation im Pflege- und Betreuungsprozess auf kommunaler Ebene vor, das insbesondere die direkte Partizipation der älteren Menschen und ihrer Angehörigen in diesen Prozess erlaubt. Dies wird durch eine Systemintegration von individuellen Möglichkeiten einer Selbstversorgung in Alltagsbereichen (z.B. über weitere Informations- und Kommunikationstechnologien) und durch Einbindung von Smart Home Systemen (z.B. pflegeplanerische Schaffung gesunder Wohn-umgebungen und betreuungs- und pflegerelevante Bedarfserhebung) möglich gemacht. Unabdingbar wird hierbei die Berücksichtigung ethisch-rechtlicher Vorgaben und Gewährleistung einer einfachen Benutzbarkeit der digitalen Innovation.

Abstract

The possibility for elderly people with care and nursing needs to lead a self-determined life at home determines the quality of life to a large extent. Supporting this need is undoubtedly a municipal task within the framework of services of general interest. In view of the demographic development, it can be assumed that over the next few years and decades the accomplishment of this task will become a great challenge for all municipalities. Since, according to forecasts, the necessary professional care and nursing resources are not guaranteed and family care is only available for a limited time (if at all) due to changing life situations, the possible applications of digital, intelligent assistive technologies must be examined more closely.

The aim of this exploratory project is to identify an innovative digital assistance system for self- and primary care of elderly people and to describe it in a specification sheet. It will then be implemented in the city of Dornbirn. The city of Dornbirn is responsible for a comprehensive elderly care system and offers extensive services for elderly people via associations and institutions in the outpatient sector (including the Nursing Association). Accordingly, many different primary and secondary users can be involved in finding solutions. The aim is to find a solution that is capable of increasing the efficiency and quality of personal care at home, shortening the length of stay for inpatient care, rehabilitation and aftercare, prolonging the self-determined life at home, promoting health and health competence and sufficiently guaranteeing individual counselling.

We have in mind a new intelligent software and hardware system for consultation, planning, control and documentation in the care process at municipal level, which in particular allows the direct participation of older people and their relatives in this process. This is made possible by system integration of individual possibilities of self-sufficiency in everyday areas (e.g. via further information and communication technologies) and by integration of smart home systems (e.g. care planning creation of healthy living environments and care-relevant needs assessment). It is essential that ethical and legal requirements are taken into account and that the digital innovation is easy to use.
