Sicheres, webbasiertes und standortunabhängiges Patientendaten- Vorerfassungs- und Nachbetreuungs-System

Projektbeschreibung

Patientendaten werden derzeit größtenteils mittels zahlreicher Telefonate oder langwieriger Papierfragebögen mit dem Patienten vorerfasst, schriftlich auf Papier festgehalten und anschließend in den Papierakten des Patienten abgelegt. Bestenfalls werden diese Informationen dann von medizinischem Hilfspersonal in die jeweilige Dokumentationssoftware manuell übertragen. Diese Daten können aufgrund der Übertragung Fehler enthalten, sind oftmals unleserlich und können auch verloren gehen. Das Suchen von Patienteninformationen ist somit oft mühsam und unnötig langsam und erhöht die Behandlungsdauer unnötig. Bei der Aufbewahrung der Patientendaten auf Papier kommen als Probleme noch die Verwechslungsgefahr und die lange Aufbewahrungspflicht von medizinischen Daten hinzu. Neben mitunter fehlerhaften Daten hat man einfach zu wenig Zeit für den Patienten selbst.
Wir möchten, dass diese Probleme mit unserem Produkt MP2.careweb ab sofort der Vergangenheit angehören, in dem Patientendaten und Fragen zum Behandlungserfolg mittels einer App für mobile Endgeräte sowie einer Web-App strukturiert abgefragt, ausgewertet und sicher aufbewahrt werden.

Unser neu entwickelter funktionaler Prototyp MP2.careweb stellt eine Applikation für Tablets dar, mit welcher Fragebögen von Patienten ausgefüllt werden können. Der Patient bekommt hierzu am Empfang ein Tablet mit dem offenen Fragebogen, welchen er ausfüllt. Das jeweilige Personal (Pflege, Therapie, Medizin) hilft bei offenen Fragen und überträgt im Anschluss den ausgefüllten Fragebogen auf Knopfdruck in das Befund- und Dokumentationssystem. Die Daten werden direkt in einer eigenen Datenbank abgelegt und in einem Software-Backend von MP2 für das jeweilige zugriffsberechtige medizinische Personal zugänglich gemacht.

Das aktuelle Produkt ist in der Ausbaustufe bis auf kleinere funktionale Änderungen und Änderungen an der Benutzeroberfläche insoweit marktreif, als dass die Patientenfragebögen beim Arzt bzw. in der Klinik auf einem mobilen Endgerät (z.B. Tablet) ausgefüllt, gespeichert und auch zentral verarbeitet werden können. Im Rahmen der Fertigstellung dieser Lösung zur Marktreife möchten wir gerne unter Verwendung des angesuchten Fördergeldes zusätzlich eine, ebenfalls als Prototyp existierende sichere Web-Applikation, zur Marktreife bringen. Diese ermöglicht eine standortunabhängige und sichere Vorerfassung der Daten durch den Patienten selbst. Eine hochsichere Schnittstelle sorgt für die Übertragung der sehr sensiblen Patientendaten zur Speicherung und Weiterverarbeitung in Drittsystemen beim Kunden. Mittels einer dynamisch generierten Web-Oberfläche können gewünschte Fragen vor, während und nach der Behandlung erhoben werden. Dazu wird vorab dem Patienten ein eindeutiger und sicherer Webzugang und parallel ein Kennwort per Post oder per SMS übermitteln. Der Patient kann sich damit sicher in unserer Lösung anmelden und die nötigen Daten für einen optimalen Behandlungserfolg mittels eines auf ihn zugeschnittenen Fragebogen ausfüllen. Um eine sichere Datenübertragung gewährleisten zu können, wird die Web-Applikation von der Datenbank abgekapselt und die Übertragung zwischen den beiden Komponenten verschlüsselt.

Das angestrebte Ergebnis ist somit die einfache, rasche und fehlerfreie Erfassung von Patientendaten und Daten zum Behandlungsfortschritt durch eine Applikation für mobile Endgeräte oder einer Webapplikation. So kann die Behandlung des Patienten bestmöglich durchgeführt werden und das medizinische Fachpersonal kann sich voll auf die Bedürfnisse der Patientinnen und Patienten konzentrieren.

Die kurze Zeitdauer, die heutzutage einer Behandlung des Patienten vorbehalten ist, kann somit sinnvoll genutzt werden. Der Patient selbst fühlt sich besser verstanden und individueller betreut, wesentliche Faktoren für den Behandlungserfolg. Die Applikation soll den Patienten vor, während und nach der Behandlung begleiten, unabhängig davon, ob diese ambulant oder stationär durchgeführt wird. Der Einstieg in die jeweilige, sehr individuelle Krankheitsgeschichte und die Darstellung des aktuellen Behandlungsstandes samt Medikation bei jeder einzelnen Behandlung wird damit in Arztpraxen, Krankenhäusern, Sonderkrankenanstalten und Kurbetrieben wesentlich erleichtert.

Die Prototypen wurden auf der Basis von Microsoft.Net-Technologien und Microsoft SQL Server entwickelt und sollen nun bis zur Marktreife fertiggestellt werden. Durch das Team von MP2 IT-Solutions soll der Applikationsserver eingerichtet, überwacht und auch gewartet werden. Neben der Absicherung des Servers nach Microsoft Standard ist die Realisierung einer zusätzlichen Web-Applikations-Firewall als weitere Schutzmaßnahme zum Schutz unbefugter Zugriffe vorgesehen. Durch externe Security-Audits (Code-Reviews und Penetration-Tests) soll eine sichere Bereitstellung des Dienstes gewährleistet werden. Zusätzliche Schnittstellen zu Drittsystemen ermöglichen den Zugang zum gesamten österreichischen Gesundheits-Markt.

Um das Produkt in den Markt einzuführen, werden wir einerseits unsere Bestandskunden in der Gesundheitsbranche vom Einsatz dieses Produktes überzeugen. Andererseits möchten wir dem gesamten österreichischen Markt diese einzigartige Lösung präsentieren, was uns insbesondere neben den herausragenden Produktfunktionalitäten durch eine Vielzahl an Schnittstellen zu bestehenden Drittsystemen gelingen soll. Neben dem persönlichen Vertrieb und dem Telefonmarketing setzen wir hierbei auf Vorstellungen der Lösungen im Rahmen von Kongressen und Informationsveranstaltungen sowie auf die Schaltung in medizinischen Fachzeitschriften. Durch eine umfangreiche Online-Werbung möchten wir insbesondere auch erreichen, dass das Produkt beim Patienten selbst bekannt und durch diesem selbst über die behandelnde Ärzteschaft nachgefragt wird.

Das angesuchte Fördergeld möchten wir einerseits für einen umfangreichen Test des Produktes mit potenziellen Neukunden, andererseits zur rascheren Steigerung des Bekanntheitsgrades im Zuge der Markteinführung nutzen.
 
Neben der zusätzlichen Web-Applikation zur standortunabhängigen Erfassung von Patientenfragebögen möchten wir die Förderung außerdem dazu nutzen, zusätzliche funktionale Anforderungen, die sich durch die Testphase mit Erstinteressenten an die Lösung ergeben, bereits in der Phase der Fertigstellung zur Marktreife berücksichtigen zu können. Weiters möchten wir mittels des Fördergeldes MP2.careweb für „third party“-Software (KIS- und RIS- Anwendung) öffnen und somit auch potenzielle Neukunden bedienen, welche bereits eine Befund- und Dokumentations-Software verwenden. Das Ziel ist eine strukturierte Erfassung des Behandlungsfortschritts, die für spätere statistische Auswertungen und medizinische Studien dienen, selbstverständlich ohne Rückschluss auf die ursprünglichen Patientendaten.

Abstract

Patient data is commonly gathered by numerous telephone calls or lengthy paper questionnaires together with the patient. The collected information is recorded in written form by hand on paper and then filed in the patients’ paper files. At the best, the patient files are then typed in by medical support staff in the respective documentation software, if such a system is existing.

This data often contain errors, the writing is illegible and the files or parts of them are lost. In addition, the slow search of patient information within the paper files prolong the actual treatment time and the untraceable information even bare the risk of unsuccessful treatments. Common problems in storing patient data on paper are the likelihood of confusion between different patients and their treatment as well as the long retention requirements by law. In regards to the occasional erroneous information one simply faces the scarcity of time for the patients themselves. MP2.careweb eliminates these risks by offering a structured patient data for successful treatment, which can be individually retrieved, evaluated, edited and securely stored in one place.

Our recently developed functional prototype of MP2.careweb offers an application for tablets which enables patients to fill out the questionnaire themselves. For this, the patient receives at the reception the tablet PC with the activated questionnaire for further adaptation. The respective staff (nursing, therapy, medicine) helps with any open issues and upon completion the information is sent by a click directly into the diagnosis and documentation system. The data will be directly stored on a separate database and made available to a back-end medical software of MP2, where the entitled medical staff has a secured access to the data.

MP2.careweb is in the final development stage and will soon reach market maturity. Currently, the basic function of the completion of patient questionnaires at the doctor or at the hospital on a mobile device (e.g., tablet), including storage and centrally editing of data are finalized and ready for further optimization. In the course of completion, we would like to use the applied grant to further develop and complete an existing prototype for a secure web application that allows on one hand, a location-independent and secure pre-capture of the data by the patient, and on the other hand a highly secure interface, which guarantees the transfer of highly sensitive patient data for storage and further processing in external systems. By means of a dynamically generated web interface the desired questions can be collected before, during and after treatment. Prior to this stage, the patient is given a unique and secure web access password transmitted by mail or SMS. The patient can safely login our web-based application and fill in the necessary information on the customized questionnaire for optimal treatment success. In addition, a structured collection of treatment progress for later statistical analysis / medical studies shall be made available. To ensure reliable data transmission, the web application must be secluded from the database and the transfer between the two components will be encrypted.
Thus the goal is to create simple, rapid, and faultless recording of patient and treatment data as well as its progress data within a framework of mobile devices and a web application in order to perform a safe and secure the treatment of the patient as well as possible and to be during the treatment to the patient, and not being able to concentrate on paperwork.
Consequently the short period of time, which is available to treat a patient, can be used more efficiently and towards the treatment itself. Therefore, the patient himself feels better understood and is treated individually, which seems to be an important aspect for a success treatment. MP2.careweb will be an active component prior, during and after the treatment regardless of whether this is an outpatient or inpatient treatment. The ability to call off the data of each very individual disease history and the representation of the current treatment including medication of each treatment is much simpler in doctors' offices, hospitals, specialist hospitals and health establishments.

The prototypes were developed based on Microsoft.Net technologies and Microsoft SQL Server and will now be completed. The team of MP2 IT-Solutions will establish, monitor and maintain the application server. After securing the server based on Microsoft standards the implementation of an additional web application firewall is provided as an additional safety measure to protect unauthorized access. Through external security audits (code reviews and penetration tests) a secure provision of the service should be guaranteed. Additional interfaces to third party systems allow access to the entire Austrian health market.

In order introduce the product to the market, we will present and convince our existing customers in the health industry on the advantages of this system. In addition, we will present this unique solution to the entire Austrian market. The variety of interfaces to existing third-party systems are in particular the outstanding product functionalities. In addition to personal sales and telemarketing we will use the framework of conferences and information sessions and advertisement within medical journals. Through an extensive online advertising, we want to achieve that the product is known by the patients themselves and demand this technology as a standard to the treatment.

The applied grant money shall be used on the one hand for a comprehensive test of the product with potential new customers, on the other hand for the use of a rapid increase of brand awareness in the market launch. Besides the additional web application for location-independent detection of patient questionnaires, we also want to develop additional functional requirements that will result from the test phase with test patients and implement them at the stage of completion of the market. We also want to apply the grant of MP2.careweb for "third party" software (HIS and RIS application) and thus serving potential new clients who already use a diagnosis and documentation software. The aim is a structured collection of treatment progress, which serves as a statistical tool for later analysis and medical studies, which is of course without any conclusion on the original patient data.
